More Than a List of Risks: What One Case Manager Wants Every Provider to Hear

“Too often, I hear in meetings feedback that providers have not responded, or that placements feel unable to meet the young person’s needs.”

That’s how an LDA Case Manager in Thames Valley described one of the most frustrating parts of her role: young people who are clinically ready to leave a Tier 4 setting, but are still waiting because no one has said yes.

For autistic young people especially, every extra week in hospital can have a lasting impact. And when a placement withdraws at the eleventh hour, months of careful preparation can be lost.

We recently supported a young woman out of hospital and into a home planned entirely around her. Her case manager was kind enough to share her reflections with us:

💜 “Working with Prosperity has been a breath of fresh air.”

💜 “Their ability to see the young person as an individual, rather than a list of risks, incidents or diagnoses, has been exceptional.”

💜 “Their approach is both compassionate and risk-positive.”

From preparing and decorating her new home, to daily meetings in the weeks before her move, to keeping her family close, every decision started with her.

A huge thank you to the case manager for taking the time to share this, and to our teams who made it happen.

If you’re supporting someone who’s ready to leave hospital, we’d welcome a conversation, however complex the referral.
